When to Use This Form
Use this form when a deceased member's heir(s) rely on a court-issued or testamentary succession document — rather than a family arrangement — to seek admission as Member.
Information You Will Need
- Original/certified copy of the succession document relied upon (Will/Probate, Succession Certificate, Legal Heirship Certificate, Letters of Administration, or Court Decree)
- Original Share Certificate of the deceased member
- Completed Indemnity Bond (Form No. 17)
