When to Use This Form
Used when a society member has deceased and their nominated heir is a minor (under 18 years). The guardian or legal representative applies on behalf of the minor for transfer of the membership, shares and flat.
Information You Will Need
- Deceased member's full name and membership details
- Death certificate of the deceased member
- Minor nominee's name and date of birth
- Guardian's name, address, occupation and income
- Share certificate number and flat details
- Undertaking for use of flat (Appendix 3)
How to Submit
