When to Use This Form
When a society member has passed away and had previously filed a nomination form, the registered nominee must apply for membership within six months of the member's death. The nominee must submit this application along with the death certificate and indemnity bond (Appendix 18 if multiple nominees).
Information You Will Need
- Deceased member's name, share certificate and flat details
- Death certificate (copy)
- Nominee's name, age, occupation and address
- Indemnity bond — Appendix 18 (if multiple nominees)
- Entrance fee ₹100
- Passport-size photograph
