When to Use This Form
Use this form when a member wants to nominate one or more persons to receive Provisional Membership of their flat/unit in the event of the member's death.
Information You Will Need
- Member's Share Certificate No. and number of shares
- Flat/Unit No. and building name
- Nominee's name, address, relationship and share proportion
- Two witnesses' names and addresses
