When to Use This Form
When a member's complaint to the society committee remains unresolved or the member is dissatisfied with the committee's response, they can file a formal grievance with the Registrar of Co-operative Societies. Common grievances include non-issuance of share certificates, refusal of membership, excess charges, audit non-compliance, non-holding of AGM, and similar matters.

- Society name and address
- Detailed description of grievance
- Previous complaints submitted to society (copies)
- Society's response (if any)
